American History Component Self-Rating Grid
Directions:
The funding source for the Teaching American History (TAH) Grant requires an annual grant evaluation. Your response to the items below will be very helpful to evaluators in assessing the impact of current grant activities on program participants. While collective group data will be shared with the program planners, your individual responses will be seen only by the evaluators and will remain confidential. The survey should be completed by May 1 of 2007.
Filling in your name and email address is necessary to verify that a survey has been received from each TAH grant participant and will not be used in any other manner. Follow-up reminders will be sent to individuals not responding to the self-rating grid according to grant evaluation timelines.
Begin the survey by completing the personal information and entering the date you are completing the form. Across the top of the grid are five levels of implementation. Please select a level of current implementation for each of the five components listed down the left side of the grid.
After reading the descriptors, find the column that best describes your overall level of implementation for EACH of the TAH components and click the button in that column. The format requires you to choose a performance level for each component each time you complete the form. Your response for each component should represent your current level of implementation at the time you complete the form.
Below
each rating please provide a brief summary of activities in which you are
currently involved to substantiate
your rating for that component. (A brief
explanation of the activity is required for each component) When typing your summaries, PLEASE do
not use a carriage return when you come to the end of the line.
